    • By the end of this project, you will have created a sample Evaluation Plan using a Mock Program. The plan will include a Logic Model with a problem statement, resources, activities, outputs, and outcomes. You will also identify measurement tools and script sample questions. The sample Logic Model and Evaluation Plan will serve as a guide to fully develop an Evaluation Plan for your selected program as well as other programs within your organization.
